Long <br />Division Director <br />RE: Permit No. M78143, I{remmling Airport, Inadequate Reporting <br />To Whom It May Concern: <br />On July 26, 1999, the Division of Minerals and Geology received the annual fee for the above <br />referenced permit. However, the Division has not received a sufficiently completed annual report <br />and/or map. Failure to submit an annual report may result in enforcement action with associated civil <br />penalties. <br />Your attention is called to the requirements of C.R.S. 34-32.5-116 (3) (a): <br />Each year, on the anniversary dare of the permit, an operator shall submit the annual fee <br />specified in section 34-32-127 (2), a report and map showing the extent of current disturbances <br />to affected [and, reclamation accomplished to date and during the preceding year, new <br />disturbances that are anticipated to occur during the upcoming year, reclamation that will be <br />performed during the coming year, the dates for beginning of active operations, and the date <br />active operations ceased for the year, if any. <br />Enclosed is an annual report form with reporting inswctions on the back. Please fill out the report <br />form and submit either a written report or an adequately labeled map as required by Rule 1.15, <br />and return it to the Division by November 1, 1999. <br />Thank you for you cooperation with this matter.
