annual fees, report, and map required by C.R.S. 34-32-116(1)(a) must be
submitted annually en or before your permit anniversary date. Fees are based
upon a schedule of E350.00 if the operation is an open mining operation and
E275.00 for any other operation. Division records indicate you oweE350.

incidate on the enclosed sheet how soon we should expect to receive the

Submittal of a report and map (to a scale of not less than 100' to the inch
and not more than 660' to the inch), indicating the phases of the reclamation
plan which have been completed and correiateu with the timetatie is requl~2d.
The report and map shall contain the following information:
1) Location and number of acres affected during that year,
2) Locations and number .of acres reclaimed for that year,
3) The location and number of acres in the various stages of
reclamation (backfilled, graded, topsoil replaced, revegetated);
4) The species of vegetation planted, the locations and approximate
dates of planting,
