In order to continue your mining operation, annual fees, report, and map are
required pursuant to sd-32-116(1)(a) of the Colorado shined Land Reclamation
Act, C.R.S 1973, as amended. Fees are based upon a schedule of $350.00 if the
operation is an open inininy operation of 10 acres or more and $275.00 for any
other operation 10 acres or more.

Submittal of a report and map (to a scale of not less than 100' to the inch
and not more than 660' to the inch), indicating the phases of the reclamation
plan which have been completed and correlated with the timetable is required.
The report and map shall contain the following information:

2) Locations ~,nd number of acres reclaimed for that year,
3) The location and number of acres in the various stages of
reclamation (b ackfil'ed, graded, topsoil replaced, revegetated),
4) The species of veyeLation planted, the locations and approximate
dates of planting,
