Questions 6 and 7 were not completed. Please finish filling out <br />the report and resubmit it to the Durango field Office for review. Please fill out all future reports <br />accurately and entirely. <br />Be aware that the annual reports are used to review compliance with the approved permit conditions <br />for each operation. The information provided must be accurate. If discrepancies or problems are <br />noted in the report, an inspection of the site may be scheduled and remedial action required. <br />As an example, this annual report indicates that no mining was conducted during the past year. Rule <br />1 .13.5 requires notice of temporary cessation to be submitted for operations that are idle for 180 <br />days or more. Please submit a notice of temporary cessation, re-activate the operation, or initiate <br />final reclamation of the site within 30 days of receipt of this notice.
