M-77-500 <br /> Dear Mr. Neesham: <br /> On July 3, 1986, the Division of Mined Land Reclamation received two annual <br /> fee payments in the amount of $50.00 each. To this date, we have no record of <br /> having received the annual reports as requested. Submission is required by <br /> the Colorado Mined Land Reclamation Act, Article 32 of Title 34, C.R.S. 1973, <br /> as amended and by the Rules and Regulations promulgated thereunder. Your <br /> attention is directed to the following provisions from Rule 2.4 for operators <br /> holding a regular, 112 permit and Rule 3.4 for operators holding a limited <br /> impact, 110 permit. <br /> 2.4 Annual Reports <br /> 2.41 On the anniversary date of each permit year, the operator shall <br /> submit a report and map (to a scale of not less than 100' to <br /> the inch and not more than 660' to the inch) , indicating the <br /> phases of the reclamation plan which have been completed, <br /> correlated with the timetable. The report and map shall <br /> contain the following information: <br /> (1 ) Location and number of acres affected during that year-, <br /> (2) Locations and number of acres reclaimed for that year, <br /> (3) The location and number of acres in various stages of <br /> reclamation (backfilled, graded, topsoil replaced, revegetated) , <br /> (4) The species of vegetation planted, the locations and the <br /> approximate dates of planting, <br /> (5) The type and approximate quantity of fertilizers, organic <br /> material or soil conditioners used and location of use , <br /> (6) An appraisal of the success of reclamation efforts, and in the <br /> event of failure of any efforts, a short narrative on the <br /> suspected or determined causes of such failure, <br /> (7) Estimates of the location and number of acres to be affected in <br /> the coming year, and <br /> (8) The effective date of the information provided above shall be <br /> no more than 45 days prior to the anniversary date. <br /> 423 Centennial Building, 1313 Sherman Street Denver, Colorado 80203 Tel.
