Box 928 <br />Meeker, CO. 81641 <br />Subject: Submittal of Undesirable Event Report: Re: White River Nahcolite <br />Dear Mr. Mehlhoff: <br />Enclosed please find Rc;port of Undesirable Event regarding wash water pipeline <br />leak on April 13, 1997. I called Mr. Paul Daggett of your staff yesterday to <br />discuss this report. The wash water line was charged, and cold temperatures the <br />week of April 7,1997 resulted in the pipeline leak. The wash water line should <br />not have been charged, following use 3-9-97, A vacuum truck was utilized to <br />remove all standing water near the spill and down slope near access road. The <br />valve in the plant has been tagged with a "Do Not Operate" tag with <br />authorization required to charge the wash water line. The wash water drain in <br />the plant will be open to signal operators if the line is charged. In addition, the <br />operators will be notifitd of this spill and informed of proper operating <br />procedures. After the soil dries, if there is noticeable surface contamination, <br />material will be disposed in the approved lined pit. Reseeding will be performed <br />if necessary.
