Headlex <br />`~ ~ <br />~I- <br />Re: File 80-135; HEADLEY FARI1S ~,,i'-x <br />Based upon our records, the anniversary date of your mining operation permit is <br />OCtOber and annual fees for the year(s) 1981, 1982 totalling <br />S are due on that date. Fees are based upon a schedule of <br />5350.00 if the operation is an open mining operation of 10 acres or more and <br />5275.00 for any other operation IO acres or rrrore. <br />• RZso, to bring the Division up to date as to the mining and reclamation on the <br />site, please submit a report and map (to a scale of not less than 100' to the <br />inch and not more than 660 to the inch), indicating the phases of the reclama- <br />tion plan which have been completed, correlated with the timetable. The report <br />and map shall contain the follotoing information: <br />1) Location and number of acres affected during that year, <br />2) Locations and number of acres reclaimed for that year, <br />3) The location and number of acres in the various stages of reclamation <br />(backfilled, graded, topsoil replaced, revegetated), <br />4) The species of vegetation planted, the locations and approximate dates <br />of planting, <br />5) The type and approximate quantity of fertilizers, organic material or <br />soil conditioners used and Location of use, <br />6) An appraisal of the success of reclamation efforts, and in the event <br />of failure of any efforts, a short narrative on the suspected or determined <br />causes of such failure, <br />7) Estimates of the location and number of acres to be affected in the <br />coming year, and <br />81 The effective date of the information provided above shall be no more <br />than 45 days prior to the anniversary date. <br />STATE OFCOLOr7AD0 R~CM~FDD U~,mrr <br />DEPARTMENT OF NATURAL RESOURCES <br />D.
