Laboratories, Inc. <br />CASE NARRATIVE <br />August 15, 2000 <br />Client: Greg Lewicki Associates <br />Date Recd: 07/17/00 <br />SDG#: L28235 <br />Lab Nos.: L28235-01 through L28235-03 <br />Sample Receipt <br />ACZ Laboratories, Inc. (ACZ) received two groundwater samples and one surfacewater sample from <br />Greg Lewicki Associates on July 17, 2000. The samples were received in good condition. Upon <br />receipt, the sample custodian removed the samples from the cooler, inspected the contents, and <br />logged the samples into ACZ's computerized Laboratory Information Management System (LIMB). <br />The samples were assigned ACZ LIMB project number L28235. The custodian verified the sample <br />information entered into the computer against the chain of custody (COC) forms and sample bottle <br />labels. <br />Sample Analysis: <br />The samples were analyzed for inorganic parameters. The individual methods are referenced on <br />both, the ACZ invoice and the analytical reports. The following anomalies were noted during the <br />analysis of these samples: <br />1. The Nitrate/Nitrite as N, dissolved analysis on sample L28235-03 was conducted using the <br />H2SO4 preserved bottle instead of the raw bottle. The samples were received on Saturday <br />and the 48 hour hold time for Nitrate/Nitrite analysis using the raw bottle was not met. The <br />data was approved since the value was less than 1 mg/L. This is a one time occurrence <br />and the correct procedure will be conducted on the raw bottle and within 48 hours on <br />future samples. <br />Holding Times: <br />All analysis was pertormed within the applicable EPA recommended holding times.
