Notice of Violation
TO WHOM IT MAY CONCERN:
The Mined Land Reclamation Act and Permit No. M-81-236 require you to submit
E50.00 in annual fees and an annual report on or before IAarch 29 of each
year. We have not received the annual fees an repor Therefore, a
violation exists to Permit No. M-81-236 and a civil penalty has been assessed
in the amount of 550.00, due and payable within 30 days of the date of this
letter. If the annual fees, annual report and the civil penalties have not
been submitted within 30 days of this notice, May 2, 1990, this permit will be
scheduled for Formal Public Hearing before the Mined Land Reclamation Board
for consideration of permit revocation and forfeiture of the 52,500.00
financial warranty at the March Board Meeting.
The May Board Meeting will be held on May 23 and 24, 1990, in Room 220 of
1313 Sherman Street, Denver, Colorado, beginning at 9:00 a.m. or as soon
thereafter as the matter can be considered and you will have the opportunity
to be represented and heard.
TO AVOID THE ABOVE ACTIONS, PLEASE SUBMIT A TOTAL OF 5100.00 (550.00 FOR
ANNUAL FEES, 550.00 FOR CIVIL PENALTIES), AND YOUR ANNUAL REPORT BY
MAY 2, 1990.
