" ..the Operator shall annually file, on the anniversary date of
the permit, a Notice of Intent to Continue (.lining Operations. The
Notice of Intent shall contain an annual fee of $50.00 a d a map or
statement that shall indicate: (a) the land affected du ing the
previous year; (b) the land to be affected during the co ing year;
