The Notice <br /> of Intent shall contain an annual fee of $50.00 and a map or statement <br /> that shall indicate: (a) the land affected during previous year; <br /> • (b) the land to be affected during the coming year, and (c) any land <br /> reclaimed during the preceding year." <br /> Based upon our records, the anniversary date of your operation is February 15 <br /> and annual fees for the year(s) 1983 totalling $ 50.00 <br /> are due on that date. <br /> If your records indicate different facts, please inform this office as soon as <br /> possible; otherwise, we will expect to receive the annual fees as required by <br /> the statute on or before your anniversary date. Also, please include the <br /> required Notice of Intent to Continue Mining Operations to bring the Division <br /> up to date as to the progress of mining and reclamation on the site.
