Banta. <br />Division Direoror <br />In order to maintain your mining operation in compliance with Colorado laws, the <br />annual fee, report, and map required by C.R.S. 34-32-116(3 and 4> and <br />34-32-127(2)(a)(II)(A-D) must be submitted annually ON OR BEFORE your permit <br />anniversary date listed below. The annual fee (this is NOT a renewal fee), report, <br />and map are for the 1991-92 mining and reclamation season, and are due and payable, <br />unless your reclamation bond has been released in its entirety, by this anniversary <br />date. <br />Division records indicate you owe $120.00. <br />Permit No. M-84-103 <br />Operation Name: Forest Service Pit <br />Anniversary Date: 02/01/92 <br />Annual Fees for the Year(s): 1992 <br />Total: $120.00 (Due on your Anniversary Date) <br />PLEASE NOTE IF YOUR ANNUAL FEES, ANNUAL REPORT, AND MAP ARE NOT RECEIVED BY THE <br />DIVISION ON OR BEFORE YOUR ANNIVERSARY DATE, A FORMAL NOTICE OF VIOLATION, AND A <br />BOARD ORDER TO PAY CIVIL PENALTIES IN THE AMOUNT EQUAL TO YOUR ANNUAL FEE AMOUNT OF <br />$120.00 WILL AUTOMATICALLY BE ASSESSED. NO PERMITS FOR NEW MINING OPERATIONS SHALL <br />BE GRANTED TO ANY OPERATOR WHO IS CURRENTLY FOUND TO BE IN VIOLATION IN THIS STATE <br />(C.R.S. 34-32-120) UNTIL THE VIOLATION IS ABATED. <br />If your records indicate different facts than those stated above or if you have <br />already forwarded your remittance, report, and map to us please inform this office <br />as soon as possible.
