Lamm <br />Governor <br />112 Annual Fee Letter April 2, 1984 <br />In order to maintain your mining operation 1n compliance with the iaw, the ~~ <br />annual fees, report, and map required by C.R.S. 34-32-116(1)(a) must be <br />submitted annually on or before your permit anniversary date. Fees are based <br />upon a schedule of f350.00 if the operation is an open mining operation and <br />f275.00 for any other operation. <br />NAME: Charles J. Webster for Estate of Ellis B. Colorado 80461 <br />ANNIVERSARY DATE: <br />ANNUAL FEES FOR THE YEAR(S): 1984 <br />TOTAL: $350.00 (Due on the Anniversary Date) <br />If your records indicate different facts, please inform this office as soon as <br />possible, otherwise, we will expect to receive the annual fees as required by <br />the statute on or before your anniversary date. Also, please include the <br />required Notice of Intent to Continue Mining Operations (Annual Report) to <br />bring the Division up-to-date as to the progress of mining and reclamation on <br />the site. If you cannot include these materials with the fees, please <br />incidate on the attached sheet how soon we should expect to receive the <br />material. <br />Submittal of a report and map (to a scale of not less than 100' to the loch _ <br />and riot more than 660' to the inch), indicating the phases of the reclamation <br />plan which have been completed and correlated with the timetable is required. <br />The report and map shall contain the following information: <br />1) Location and number of acres affected during that year, <br />2) Locations and number of acres reclaimed for that year, <br />3) The location and number of acres in the various stages of <br />reclamation (backftlled, graded, topsoil replaced, revegetated), <br />4) The species of vegetation planted, the locations and approximate <br />dates of planting, <br />423 Centennial Building, 1313 Sherman Street Denver, Colorado 80203 Tel.
