All <br />data gioen, such as pro~uction rates, topsoil removal, etc., should be <br />for that period of 12 r.,onths that is expected to have the greatest <br />amount cf activit}', usually:for the first year. En;ine=_rin? or nar.u_`acturing data on control eauicntnt, <br />PERh:ITS REpU?RSD <br />Fugitive Particulate Emission Permits are issued for site-specific <br />activities, such as mining, storage of materials, haul road activities, <br />etc. <br />The Division will use th=_ information submitted on this form to estivate <br />emissions from the acti-.~it}. <br />Any orocessina equipment, such as a crusher, screen decY„ or asphalt <br />plant is considered to be portable and requires a separate eermit <br />application, Air Polluticn Emission iJOtice, and filing fee. <br />This aids the applicant by requiring only a revised Air Pollution <br />Emission piotice for relocation, whenever the equip,-~ent is moved to a ne~~r <br />site. <br />FEES <br />A filing fee of $100 shall accompany each Air Pollution Emission notice <br />/ Control Plan filed with a permit application. Permit processing fees <br />will be charged for the a~;,ount of time the Division spends evaluating <br />the permit application. These fees, (currently $50 per hour) plus any <br />required public notice fees, must be paid before an Initial Approval <br />Permit Brill be issued. one=_ the project has commenced operatics, an <br />inspection will be conducted by the Division. If all permit conditions <br />are met, a Final Approval F=_rmit will be recommended. Final Approval <br />Permit processing fees dust be paid before the Final F.opro•;al Perr.~it is <br />issued. <br />00047 <br />
