In order to continue your mining operation, annual fees, report, dni~yfiap bre
required pursuant to 34-32-11ti(1)(a) of the Colorado i~tined Land Reclama~:ion
Act, C.R.S 1973, as amended. Fees are based upon a schedule of $350.00 if the
operation is an open mininy operation of 10 acres or more and $275.00 for any
other operation 10 acres or more.

If your records indicate different facts, please inform this office as soon as
possible, otherwise, we will expect to receive the annua] fees as required by
the statute on or oefore your anniversary date. Also, please include the
required Notice of Intent to Continue ~4ining Operations iAnnual Report) to
wring the Division up-to-date as to the progress of mining and reclamation on
the site. If you cannot include these materials with the fees, please
incidate on the attacnea sheet how soon we Should expect to receive :he
material.
Suomittal of a report and .nap (to a scale of not less titan 1'JO' to the inch
and not more than tib0' to the inch), indicating the phases of the reclamation
plan which have been completed and correlated with the timetaole is required.
The report and snap shall contain the following information:
1) Location and number of acres affected during that year,
Z) Locations and numuer of acres reclaimed fur that year,
3) The location and numuer of acres in the various stages of
reclamation (backfilled, yraded, topsoil replaced, re vegetated),
1) The species of vegetation planted, the locations and appr~xinate
dates of planting,
