ANNUAL REPORT <br />MONOGRAM MINES <br />PERMIT M-1978-222=UG <br />February, 2002 <br />14ECEIVED <br />~2 ~2~ <br />Oirisian of Minerals and Geology <br />The Monogram Mines Permit was inspected by the Durango Field Office of the <br />Colorado Division of Minerals & Geology on June 4, 2001. The Inspection Report <br />identified Corrective Action items relating to Permit identification signs, Permit <br />boundary markers, and current reports and maps of affected aeeas covered by the Permit. <br />A response to the June 4, 2001 inspection was submitted to the Grand Junction <br />Field Office of the Division of Minerals & Geology on November 29, 2001. A copy of <br />the text of the November 29 response is attached hereto. Photographic evidence was <br />provided of the installation of a Permit identification sign. Anew map was also provided <br />with the November 29 report to the Grand Junction Field Office. The photographs and <br />map are on file with the Grand Junction Field Office. <br />A follow up inspection was conducted by the Grand Junction Field Office of the <br />Division of Minerals & Geology on November 27, 2001. According to the November 27 <br />Inspection Report, the Corrective Action items pertaining to Permit identification signs <br />and boundary mazkers remain open. The inspector has proposed that the operator and the <br />Division of Minerals & Geology conduct a joint field inspection in Spring 2002 to <br />attempt to determine and agree upon the affected areas that are the responsibility of the <br />operator under the current Permit. <br />No reclamation work was conducted during the past year on this Permit. No new <br />disturbance activities or reclamation work aze planned during 2002. <br />
