Box 5485 <br />Denver, Colorado 80217 <br />Re: Annual Report, Lawrence Pit, File No. M-74-041 <br />Dear Mr. Ballard: <br />We received your submittal of annual fees on January 21, 1988. However, no <br />annual report was received as required by C.R.S. 34-32-116(a). This report <br />need not be an extensive document, but rather a response to eight (8) <br />statements as listed in Rule ?.4 below: <br />2.4 Annual P.eports <br />2.41 On the anniversary date of each permit year, the operator shall <br />submit a report and map (to a scale of not less than 100' to the <br />inch and not more than 660' to the inch), indicating the phases of <br />the reclamation plan which have been completed, correlated with the <br />timetable. The report and map shall contain the following <br />information: <br />(1) Location and number of acres affected during that year, <br />(2) Locations and number of acres reclaimed for that year, <br />{3) The location and number of acres in various stages of <br />reclamation (backfilled, graded, topsoil replaced, <br />re vegetated), <br />(4) The species of vegetation planted, the locations and the <br />approximate dates of planting, <br />(5) The type and approximate quantity of fertilizers, organic <br />material or soil conditioners used and location of use, <br />(6) An appraisal of the success of reclamation efforts, and in <br />the event of failure of any efforts, a short narrative on the <br />suspected or determined causes of such failure, <br />(7) Estimates of the location and number of acres to he affected <br />in the coming year, and <br />(8) The effective date of the information provided above shall be <br />no more than 45 days prior to the anniversary date. <br />Also, please note that a map is also required to he submitted as part of the <br />annual report. <br />423 Centennial Building, 1313 Sherman Street Denver, Colorado 80203-2273 Tel.
