In order to continue your mining operation, annual fees, report, and map are
required pursuant to 34-32-11b(1)(a) of the Colorado f~lined Land Reclamation
Act, C.R,S 1973, as amended. Fees are used upon a schedule of $350.OU if the
operation is an open ~nininy operation of 10 acres or more and X275.00 for any ~G
other operation 10 acres or more.

• If your records indicate different facts, please inform this office as soon as
possible, otherwise, we will expect to receive the annual fees as required by
the statute on or before your anniversary date. Also, please include the
required Notice of Intent to Continue 14ining Operations (Annual Report) to
wring the Division up-to-date as to the progress of mining anc reclamation on
the site, If you cannot include these materials with the fees, please
incidate on the attached sheet how soon we should expect to receive the
material,
Submittal of a report and map (to a scale of not less than lOfi' to the inch
and not more than 6ti0' to the inch), inaicating the phases of the reclamation
plan whim have been completed and correlated with the timeta„le is required.
The report and map shall contain the following information:
1) Location and number of acres affected during that year,
2) Locations and number of acres reclaimed r"or that year,
3) The location and number of acres in the various stales of
reclamation (backfilled, yraded, topsoil replaced, revegetateri),
4) The species of vegetation planted, the locations and approximate
dates of planting,
