TO WHOM IT MAY CONCERN:
Section 34-32-116(3) and (5 ), C.R.S. (1989 Supp.) and Permit No. M-82-129
require you to submit $50.00 in annual fees and an annual report on or
before August 05 of each year. As of the date of this letter we fia`ve not
received your annual fees and report. Therefore, you are in violation of the
above cited statute and the provisions of your Permit No. M-82-129. Pursuant
to Section 34-32-124(7), C.R.S. (1984) you are hereby assessed a civil penalty
in the amount of 550.00.
If you fail to submit your annual fees, annual report and the civil penalties
assessed to our office by October 4, 1990, this matter will be scheduled for
Formal Public Hearing before the Mined Land Reclamation Board for
consideration of permit revocation and forfeiture of your 1;5,000.00 Deed of
Trust at the October Board Meeting.

E50.00 FOR C OTAL OF X100.00), AND YOUR ANNUAL REPORT BY
OCTOBER 4, 1990.
The Board Meeting will be held on October 17 and 18, 1990, in Roam 220 of
1313 Sherman Street, Denver, Colorado, beginning at 9:00 a.m. or as soon
thereafter as the matter can be considered, and you will have the opportunity
to be represented and heard.
