(1989 SupD.) and Permit No. M-82-086 <br />require you to submit E350.00 in annual fees and an annual report on or <br />before June 11 of each year. As of the date of this letter we have not <br />received your annual fees and report. Therefore, you are in violation of the <br />above cited statute and the provisions of your Permit No. M-82-086. Pursuant <br />to Section 34-32-124(7), C.R.S. (1984) you are hereby assessed a civil penalty <br />in the amount of 5350.00. <br />If you fail to submit your annual fees, annual report and the civil penalties <br />• assessed to our office by August 1, 1990, this matter will be scheduled for <br />Formal Public Hearing before the Mined Land Reclamation Board for <br />consideration of permit revocation and forfeiture of your <br />$8,400.00 Certificate of Deposit at the August Board Meeting. <br />YN ORDER TO AVOID PERMIT REVOCATION, PLEASE SUBMIT 5350.00 FOR ANNUAL FEES, <br />5350.00 FOR CIVIL PENALTIES (FOR A TOTAL OF 5700.00), AND YOUR ANNUAL REPORT <br />BY AUGUST 1, 1990. <br />The Board Meeting will be held on August 22 and 23, 1990, in RDDm 220 of <br />7313 Sherman Street, Denver, Colorado, beginning at 9:00 a. m. or as soon <br />thereafter as the matter can be considered, and you will have the opportunity <br />to be represented and heard. <br />If you have any questions, please contact Barbara Chiappone. <br />Sincerely, <br />~~ <br />FRED R.
