Fees are based upon a <br />schedule of E350 if the operation is an open mining operation and 5275 for any <br />other operation. The <br />report and map shall contain the following information: <br />1) Location and number of acres affected during that year; <br />2) locations and number of acres reclaimed for that year; <br />
