Long <br />Division Dimctor <br />110(2) ANNURI fEE INUOICE and REPORT REQUEST <br />PLEASE READ CAREFULLY <br />Annual fees are not permit renewal fees, they are fees for having a valid <br />reclamation permit, and must be paid each year until reclamation responsibility <br />release is granted. If you have requested reclamation release from the Division <br />but your permit is not terminated by your anniversary date listed below the annual <br />fee MUST 6e paid. If the Division does release this permit before the anniversary <br />date please do not pay this fee. <br />• The fee is for LAST YEARS mining and reclamation season, and MUST be paid even if <br />the mine was in an inactive state. It is due and payable ON OR BEFORE the permit <br />anniversary date shown below. <br />PLEASE NOTE: IF THE ANNUAL FEE, ANNUAL REPORT, AND MAP ARE NOT RECEIVED ON OR <br />BEFORE THE ANNIVERSARY DATE, A FORMAL NOTICE OF VIOLATION, AND A BOARD ORDER TO P <br />CIVIL PENALTIES IN THE AMOUNT EQUAL TO THE ANNUAL FEE OF $225.00 WILL BE <br />AUTOMATICALLY ASSESSED.
