In order to maintain your mining operation in compliance with the law, the annual
fees, report, and map reauired by C.R.S. 34-32-116(3) and (4) must be submitted
annually on or before your permit anniversary date. Fees are based upon a
schedule oT-1330—7—your operation is an open mining operation and $275 for any
other operation. Division records indicate you owe $350.00.

fee, the enclosed Notice of Intent to Continue Minino Operations (your annual
report), and your map, as reauired by the statute, on or before your anniversary

enclosed annual report form how soon we should expect to receive these materials.

To bring the Division up-to-date as to the reclamation on the site, please include
in your annual report, and map the following information:
Your map should be to a scale of not less than 100' to the inch and not more than
660' to the inch, it should indicate the phases of the reclamation plan which have
been completed and correlated with the timetable as reauired. Your report and map
shall contain the following information:
• 1 ) Location and number of acres affected during that year;
2) locations and number of acres reclaimed for that year;
