Long <br />Drvrsion Director <br />Annual fees are not permit renewal fees, they are fees for having a valid <br />reclamation permit, and must be paid each year until reclamation responsibility <br />release is granted. The fee is for last year even if the mining operation was in <br />an inactive state. It is due and payable ON OR BEFORE the permit anniversary date <br />shown telow. <br />• <br />PLEASE NOTE: IF THE ANNUAL FEE, ANNUAL REPORT, AND MAP ARE NOT RECEIVED ON OR <br />BEFORE THE ANNIVERSARY DATE, A FORMAL NOTICE OF VIOLATION, AND A BOARD ORDER TO PAY <br />CIVIL PENALTIES IN THE AMOUNT EQUAL TO THE ANNUAL FEE OF $490.00 WILL BE <br />AUTOMAIICALLY ASSESSED. NO PERMITS FOR NEW MINING OPERATIONS SHALL BE GRANTED TO <br />ANY OPERATOR WHO IS CURRENTLY FOUND TO BE IN VIOLATION IN THIS STATE <br />(C.R.S. 34-32-120) UNTIL THE VIOLATION IS ABATED. <br />The Division credited the $175.00 annual fee payment from Permit No. M-89-096, <br />which Maas received on December 9, 1992, to a portion of this Permit.
