Applegate Group. Inc. reque is a roval ofthe enclosed Gravel Pit <br />Well Permit Application for the Duckworth Pit sand and gravel mining operation. A check made out <br />to the Division of Water Resources for the 5480.00 well permit fee is enclosed. The substitute <br />supply plan for the site was submitted to the SEO on March 17, 2004. The projected start date for <br />the mining operation is July 2004. <br />The. Duckworth Pit is located along Boulder Creek southeast of Longmont in Section 16; T2N, <br />R68W, 6a' P.M. Figure No. 1 is a vicinity map of the Duckworth Pit mining site. Figure No. 1. also <br />shows the location of the wells identified by the SEO's Well Look database. Four of the wells <br />shown in Figure No. I are owned by Lafarge with three being monitoring wells. <br />Water Commissioner Bob Carlson completed a 600' well spacing inspection on March 16, 2004. A <br />copy of his report is enclosed. He determined that there were no wells located within 600 feet ofthe <br />pit.
