On behalfofLafarge West, Inc.. Applegate Group. Inc. reque is a roval ofthe enclosed Gravel Pit
Well Permit Application for the Duckworth Pit sand and gravel mining operation. A check made out
to the Division of Water Resources for the 5480.00 well permit fee is enclosed. The substitute
supply plan for the site was submitted to the SEO on March 17, 2004. The projected start date for
the mining operation is July 2004.
The. Duckworth Pit is located along Boulder Creek southeast of Longmont in Section 16; T2N,
R68W, 6a' P.M. Figure No. 1 is a vicinity map of the Duckworth Pit mining site. Figure No. 1. also
shows the location of the wells identified by the SEO's Well Look database. Four of the wells
shown in Figure No. I are owned by Lafarge with three being monitoring wells.
Water Commissioner Bob Carlson completed a 600' well spacing inspection on March 16, 2004. A
copy of his report is enclosed. He determined that there were no wells located within 600 feet ofthe
pit. The 30-day notification process will not be required.
