THE SPECIFIC EQUIPMENT OR ACTrirITY SUBJECT TO THIS PERMIT INCLUDES THE FOLLOWING:
Mine ventilation system e~austing through one (1) Joy, Model: M72-43D, SIN: MF 5736,
mine venU7ation fan, design rated at 150,000 ACFM.

IN THIS DQCUMENT AND THE FOLLOWING SPECIFtc TERMS ANO CONDffIONS:
1. AR previous versions of this permit are canceled upon issuance of this permit.
2. Visible emissions shall not exceed twenty percent (20%) opacity during norrr~l operetion of the
source. During periods of startup, process modification, or atljustment of control equipment
visible emissions shah not exceed 30°1° opaaty for more than sbr minutes in any sixty consecutive
minutes. Opadty shall be measured by EPA Method 9. (Reference: Regulation 1, Section
II.A.1.& 4.)
3. This source shalt be limited to a maximum production rate as fisted bebw and aB other activities,
operational rates and numbers of equipment as stated in the application. Annual records of the
actual production rate shall be maintained by the applicant and made availatNe to the Division for
inspection upon request (Reference: Regulation 3, Part 6, III.A.4)
Production of coal shall not exceed 1,200,000 tons per year.
4. The permit number sha0 ba marked on the subject equipment for ease of identification.
