Long, <br />Division Director <br />On March 25, 1992, the Mined Land Reclamation Division received the 112 <br />application package for the Sand Creek Farms, Inc., Sand Creek Mine. Please <br />be advised that the following Exhibits did not meet the full requirements of <br />the Colorado Mined Land Reclamation Rules and Regulations and the Colorado <br />Mined Land Reclamation Act. Please resubmit these Exhibits so that the <br />Division may begin the technical review of the application. <br />EXHIBIT B - Index Map: (Rule 2.1.2(3)) An index map showing the <br />regional location of the affected land and all roads and other access to <br />the area. A standard U.S. Geological Survey topographic quadrangle or <br />equivalent is acceptable. Scale criteria need not be followed for this <br />map. <br />EXHIBIT F - Reclamation Plan Map: Please consult Rules 2.1.2(1) and <br />2.1.2(7)) for information necessary to complete Exhibit F. Be sure to <br />submit a map with contour lines of sufficient detail to portray the <br />proposed direction and rate of slope of all reclaimed lands. Also, any <br />mine plan and reclamation plan maps should clearly show the proposed <br />permit area boundaries and at least one easily locatable point. <br />EXHIBIT L - Reclamation Costs: Please consult Rule 2.1.2(13) for <br />information necessary to complete Exhibit L. Please keep in mind the <br />cost estimation should be figured as if a third party contractor would <br />reclaim the land under contract with the State of Colorado. <br />EXHIBIT N (Shown as a second Exhibit L in Rules and Regulations) - Other <br />Permits and Licenses: Please consult Rule 2.1.2(14) for information <br />necessary to complete Exhibit N. You may need to contact the county to <br />start a mining operation in this location. You may also need to contact <br />the Army Corps of Engineers to obtain a Section 404 Dredge and Fill <br />permit. You may also need to obtain a fugitive dust emissions permit <br />from the State of Colorado Department of Health, Air Quality Control <br />Division. <br />STA'~ OF COLORADO <br />
