4, The Applicant shall submit to the Division for approval aground water <br />monitoring program as a modification to the permit prior to initiation of any blasting or <br />earth-moving activities at the site and prior to any excavation exposing groundwater at <br />the site. The program shall contain all components specified in Construction Materials <br />Rule 3.1.7(7)(b). At a minimum, the monitoring program shall include four monitoring _ <br />wells upgradient of the pit and four monitoring wells downgradient of the pit drilled to <br />the proposed final depth of excavation. An additional monitoring well shall be drilled <br />approximately mid-way along a line between the centroid of the pit and the Black Hawk- <br />Central City Sanitation District well. The nine monitoring wells shall be drilled by the <br />Applicant prior to any blasting or other earth-moving activities and prior to any <br />excavation exposing groundwater at the site. Applicant shall have a geologist on site <br />during the drilling of each of the monitoring wells who shall describe the cuttings during <br />the drilling of the monitoring wells in a written report for each well, and the Applicant <br />shall provide each report to the City of Black Hawk and the Black Hawk-Central City <br />Sanitation District along with a written summary of how the wells were completed, <br />including casing size, size and depth of casing slots, and details of cementing and well <br />development. The constituents to be sampled or measured at each of the monitoring <br />wells include: radioactive pazameters (including gross alpha, radium 226 and radium 228, <br />uranium, thorium 230 and 232), and turbidity, and ground water levels. The monitoring <br />wells shall be sampled monthly and ground water levels shall be measured monthly for a <br />minimum of four months prior to initiation of any blasting or earthmoving activities at <br />the site and prior to any excavation exposing groundwater at the site. Monthly sampling <br />and measurement of the groundwater levels in the eight monitoring wells shall continue <br />until completion of the reclamation and final release of the reclamation bond. The <br />monitoring results shall be reported monthly to the Office of the State Engineer, Colorado <br />Department of Health and Environment, the City of Black Hawk Water Department, the <br />City of Central Water Department, and the Black Hawk-Central City Sanitation District. <br />Drill cuttings from the construction of the monitoring wells shall be monitored and <br />logged for radioactivity. In addition, the Applicant shall agree to conduct and pay for <br />monthly monitoring of the Black Hawk-Central City Sanitation District Wastewater <br />Treatment Plant well for all above-described pazameters, including groundwater levels. <br />If an exceedence of water quality standards is documented in any sample of surface or <br />ground water that can or will leave the property boundaries, upon becoming aware of the <br />test results, the Applicant shall immediately report the results to the Colorado Department <br />of Public Health and Environment by telephone. The Applicant shall resample the source <br />for the parameter(s) in violation, requesting an expedited turn azound time for the second <br />sample results. Within five (5) working days of the initial phone notification, the <br />Applicant shall submit in writing to CDPHE, with a copy to the Division, a copy of the <br />sample results, and the following information: a description of the water quality standazds <br />exceedence and what likely caused it; the period of noncompliance with standazds, <br />including dates and times andlor the anticipated time when the discharge will return to <br />compliance; and all steps being taken to reduce, eliminate, and prevent recun•ence of the <br />noncomplying discharge. <br />2 of 4 <br />
