Preliminary review of the <br />information received determined that the following items muss be received before the Division can consider your <br />apphcation as being submitted and technical review can begin. Please be advised that all exhibits must be addressed.: <br />APPLICATION FORM: The application form has beer) determined to be incomplete. Please complete and <br />retlun. Page l: The Division cannot accept the statemetlt "Less than I O", }'ou must be specific. Please list <br />the exact acreage that is to be permitted and the .oral acreage N the permit area. Page 4 & ~ - <br />Responsibihties as a Permittee: Please complete. Pigs 6 -Please afTir Wagner Construction Inv.'s <br />Corporate Seal where highlighted in blue. <br />EXHIBIT B -Site Description (Rule 6.3.2(al): Ycu must address Item (a) to the extent necessary to <br />demonstrate compliance with the applicable performana: standard requvements of Rule 3. At a minimum. <br />you must include a description of the vegetation ani soil characteristics in the azea of the proposed <br />operation. The local of5ce of the Soil Conservation Se.1•ice (SCS) may provide you with this information <br />as well as recommendations for Exhibit D -Reclamation Plan; <br />EXHIBIT B -Site Description (Rule 6.3.2(bl): You must address Item (b) to the extent necessity to <br />demonstrate compliance with the applicable performanc: standard requirements of Rule 3. At a minimum. <br />you must identify any permanent man-made structures within nvo hundred (200) feet of the affected area <br />and [he owner of each structure. Permanent man-madly structures include houses, barns, fences, above or <br />below ground utilities, irrigation ditches, maintained or public roads, bridges, wells, pipelines. water storage <br />impoundments, railroad tracks, cemeteries. and communication antennas, etc. Each swcture should be <br />located on Exhibit E -Map; <br />EXHIBIT B -Site Description (Rule 6.3.2tc11: You must address Item ~% to [he extent necessary to <br />demonstrate compliance with the applicable performano: standard requirements of Rule 3. At a minimum. <br />you must give a description of the water resources ire the area of the proposed operation.
