~.. <br />~]. <br />~' <br />st: <br />• <br />AGILE STONE SYSTEMS, INC. <br />October 29, 1997 rf~ <br />~. <br />INSPECTION OF HIGHWAY 50 BRIDGE <br />SPANS OVER ARKANSAS RIVER <br />PARKDALE,COLORADO <br />OCTOBER 16,1997 <br />Puruose <br />This investigation was conducted to document the existing condition of the Highway 50 <br />bridge that spans over the Arkansas River in Pazkdale, Colorado. <br />Procedure <br />The investigation began with a meeting near the bridge. Present at the meeting were Ken <br />Kclo (Azurite Inc.), Phil Musial and.Ben Carroll (both with Vibra-Tech Engineers). The <br />top, sides, underneath and footers of the bridge were viewed. <br />• The condition of the inspected sections of the bridge may change after the date of <br />inspection due to many factors such as weather, fire, moisture, actions taken by the <br />Colorado Department of Transportation, or the passage of time. This report deals with <br />the condition of the bridge at the time of inspection. <br />The following report is based upon an examination of only those portions of the bridge <br />that were readily accessible and could be visually inspected from the ground. No <br />_ inspection was made in areas obstructed, concealed, closed off; or in any azea which <br />required dismantling, removal or movement of any object. <br />Photographs are attached to the rear of this report. <br />Roll #1 <br />Photo # 1) Identification photo from the east. <br />Photos # 2-7) Identification photos of southern most beam, east side of river. <br />Photo #8) East side, mid beam -concrete crack. <br />Photo #9) East side, southern most beam - no expansion pad. <br />• Photos #10-13) East side, mid column -concrete crack. <br />
