It can be in the form of metes b bounds survey or a des~rip- <br />tion 6y reference to township, range, and section, to at leas[ the nearest quarter- <br />quarter section. Where applicable, the street address or lot number(s) my be used. <br />ERHffiIT B - HininR Plan <br />This can be a general discussion of the methods, equipment, phases or stages of the <br />operation and those portions of land affected by each. The plan should be <:orrelated <br />with the map and should shoe where ezeraction will begin, and which direction, if any, <br />it will proceed. <br />L7CFiffiIT C - Mao <br />This map should clearly define permit boundaries and indicate all roads or stockpile <br />areas to be used Sa connection with the operation. AlI immediately adjacent 1.andovners <br />must be show. The quarter section corner, section line., or nay other locateable <br />points should also be show on this map. <br />ERRIBIT D - Wildlife, Water Resources, Vegetation and Soils Information <br />Each of the above-mentioned subjects must be addressed as outlined iu the applicable <br />RULE. <br />ERAIBIT E - Reclamation Plan <br />As with the mining plan, this should be a general discussion of the methods, equipment, <br />phases or stages of the reclamation process and chose portions of land affected by each. <br />If possible, the plan should be closely correlated with the map. If not, tine narrative r <br />must be comprehensive enough co draw as accurate verbal .picture of the e.~cpected appeaz- <br />ance of the area after completion of reclamation. <br />E~iffiIT F - Proof of Local Government Approval <br />This must be a letter from the local government specifying conformance with existing . <br />zoning regulations. (It is intended that operators may~abtaia local gove rraent approval <br />concurrent vi th the-Divlsion's processing of the-app`lieatlon. To do so, the .iperacor <br />must submit evidence which indicates that local government approval is currently pending <br />or will a[ least be considered within a reasonably short time by the appropriate govety- <br />Sng body R1JI.E 1.1(10) (Consult this RULE for full details an this requirement.) <br />F%HIBIi G - Source of Leaal Right to Enter <br />This could be a copy of the lease, deed, abstract of title, or current tax receipt. <br />An acceptable alternative would be a statement acknowledged by a notary public that <br />the operator has the legal right to enter and mine. <br />EXAffiIT R - Reclamation Costs <br />For a 120 permit, if the operator recuests surety to be less than 52500, the onera[orausc~ <br />submit facts to support such a request. Otherwise, a surety of 52500 will automatically <br />be set. For all 111 permits, the surety shall be 52500 per acre. <br />E!OiffiIT I - Terms of Governmental Contracts <br />For Special 10-Day (111) Permits, [he operator must submit a copy of the covtraet or <br />ocher proof [ha[ proves the necessity of a special 10-day permit. The operator must <br />also submit evidence of the surety provided under the governmental contract, Sf it vas <br />required. <br />NOTICE PROCEDURES <br />After [he application is considered "filed" (pursuant to RULE 1.1(10) with ih~e `tined <br />Load Reclamation Division, notice procedures otatl hied in R:;LES 3.2 and/or 4.2 must be <br />followed.
