The Division is returning the Application <br />for Succession of Operators for the following: <br />The Division requires that a Transfer of Approved Mineral Application form be submitted rather than <br />the form originally submitted. <br />The fee for a transfer is $125.00, therefore, the Division requires that an additional $25.00 be submitted. <br />The Division is also returning Page 1, 3, 5, 6 and the Signature page of the 112c permit application form for the <br />following: <br />The Division requires that the Applicant/Operator names remain consistent throughout all documents <br />(i.e. Diana K. Watson vs. Diana K.
