THIS PERMIT is issued by the Mined Land Reclamation Board, Departmart of Natural Resources,
State of Colorado.
RECITALS
A. The PemriVee, City of Greeley desires to conduct a mining operation lmown as 25th Avenue
Site, for the purpose of extracting sand and gravel The Pemuttee will not mine any secondary commodities.
B. On May 30, 2002 the Mined Land Reclamation Board (the "Board") approved the Pemrittee's
apphcation for this permit and drrected that this permit be issued upon the filing with the Divisicn of Minerals and
Geology (the "Division") of performance warranty approved by the Division. Said warranty has been filed with
the Division.
C. If the Pemtittce desires to extract materials other than those listed in (A), a separate pemrit may
be regrdred.
D. On May 30, 2002 the Board made the following findings:
1. The application for this permit corr>plies with the requiremarts of the Colorado Mined
Land Reclamation Ad forthe Extraction of Constnrction Materials, C.RS. 3432.5-101 ~ sue., as amended, and
wick all applicable local, state and federal laws;
2. The operation will not adversely affect the stability of any significant, valuable, and
permanent man-made stmchrre located within two hundred feet of the Affected Land, except where there is an
agreement betwear the Operator and the persons having an interest in the structure that damage to the structure is
to be compensated for by the Permittee; and;
