Clipped separately, you will find documents that should be helpful. I have included a copy of the sign that was posted at the current entrance to the pit azea and at the proposed entrance. You will also find the documents that show ownership of the land, and those showing the change in my marital status and name change. There is a transfer of permit and succession of operators application form that has been filled out and was signed by Darryl Stout at the time of our divorce. The check for the fee is clipped to this application. I have included one map that has had the adjacent landowner names added to it. There are no other changes from the maps you have on file. As it is difficult for me to get adequate copies and the additions are simple, I have only included the one copy. I have addressed each item in the adequacy review comments. Each one is referenced in the same numerical order and with the applicable rule documented. You will also find a letter from the Colorado State Extension Agent, Bill Ekstrom, reviewing his recommendations for revegitation. I have included copies of required documents with the original and a set of 4 separate copies (five total) of the application and other documents such as the updates addressing the adequacy review comments. Following the publication of the last legal notice in the local newspaper, I will send a copy of the three notices as they appeared in the paper.
