This report is to be <br />submitted within 30 days after the end of the period of exploration specified in the <br />approved Notice of Intent, as amended. <br />1. Attach a map showing: <br />a) The final locations of all holes drilled during this project; See attached map <br />b) the locations of all roads modified or constructed for the project; and None <br />c) All areas significantly disturbed by the operator. Noae <br />2. Have all disturbed areas, including drill holes, been reclaimed in accordance with the <br />approved plan and the above-referenced regulations? Yes <br />3. Were any newly constructed project roads retained for the landowner? No <br />If yes, a) attach a letter from the landowner that grants consent, b) indicate the road <br />location on a map, and c) describe the approximate specifications of the road (width, <br />length, surface material used). <br />
