Box KK <br />Meeker, CO 81641 <br />RE: Notification of Board of Commissioner final approval. <br />Dear Mr. Ullom: <br />At the regularly scheduled meeting of the Rio Blanco County Board <br />of Commissioners held Tuesday, May 26, 1998, at 1:15 p.m. the Board <br />voted unanimously to approve your Special Use Permit #98-22. Approval <br />is subject to the following conditions: <br />1. Weeds be adequately controlled in all disturbed areas <br />2. A Fugitive Particulate Emission Permit be obtained from the Colorado <br />Department of Health. <br />3. A Mined Land Reclamation Permit be obtained from the Mined Land <br />Reclamation Division of the Colorado Department of Natural Resources. <br />4. Appropriate sediment control measures be taken to ensure that <br />sediment will not reach adjacent drainages or properties. <br />5. Dust will be controlled to regulated levels at all times on the entire site <br />and access road. Operations will cease immediately if dust cannot be <br />adequately controlled. <br />6. Any asphalt plant located on this site shall be located as far away as <br />possible from County Road 27 or otherwise in a location to minimize <br />impacts to adjacent properties. <br />7. Any mud tracked onto County Road 27 will be removed by the operator <br />of the pit on a daily basis during hauling periods. <br />8. Disturbed areas where mining operations have been completed will be <br />reclaimed on an annual basis. Upon completion of mining operations, <br />the area shall be recontoured and smoothed to slopes no steeper than <br />3:1 and preferably flatter. The area will then be covered with topsoil to <br />a minimum depth of 6-inches and reseeded. <br />
