On October 10, 1997, the Division of Minerals and Geology received First National Bads Time Certificate of Deposit No.
70712, in the amount of $5,000.00, which is being submitted as a collateral reclamation bond on behalf of Stone Wholesale
Corporation. The Division cannot accept Certificate of Deposit No. 70712 for the following reasons:
The address and phone number on the CD must be the Division's address and phone number. Our address and
phone number is 1313 Sherman Street, Room 215, Denver, CO 80203; [phone]. This must be stated on
the CD itself, any signature cards, and also on the books of the bank. The reason for this is so that the Division
can track the CD. All interest checks, interest statements, 1099 fortes, and any other information pertaining to
this certificate is supplied to the Division then the Division will either copy the information to Stone Wholesale
Corporation or send the original information to Stone Wholesale Corporation.
Certificate of Deposit /pterest: The interes[ shodd not accrue against the Cer[ifica[e. The interest does belong to
Stone Wholesale Corporation and shodd be either deposited inm a checking or savings account, or paid by check
to Slone Wholesale Corporation. [f Stone Wholesale Corporation world like to receive the interest by check, you
shodd mail the check to the Division and the Division Director will endorse the check over to Stone Wholesale
Corporation. This is a way for the Division to keep vack of the CDs to safeguard against the premature cashing
of each certificate.

signature cazd to be signed please send Ote cazd to the Division. The Division Director is the ody approved
signature required for endorsement of the certificate.
The State requires the bank make the above memioned changes to Certificate of Deposit No. 70712, which is enclosed, to
safeguard against the premature cashing of the certificate. 1 have enclosed a copy of the instruction sheet for Certificate of
Deposits.
