Due to Mineral Rules and Regulation changes in 1994 and <br />new changes that became effective May 1, 1995, preliminary review of the <br />application determined that the following items must be submitted before the <br />application can be considered submitted and technical review can begin: <br />APPLICATION FORM and NOTARIZATION: The application form that you submitted <br />isoutdated and has not been used since 1994. 2 have enclosed the current <br />112 application package for your use (copies of this form may be made). <br />Application forms change each time the Rules and Regulations are amended <br />(this form will probably change in July or August of 1995 sue to the new <br />rule changes that became effective May 1, 1995). <br />Please submit an original notarized (the first application package that you <br />submitted was not notarized) application and four copies to the Division. <br />Rule 1.6.2(1)(b) (Effective Mav 1, 1995): You must post notices at the <br />location of the proposed mine site, of sufficient size and number to <br />clearly identify the site as the location of a proposed mining operation <br />giving your name, address, and phone number, and stating that you have <br />applied for a mining permit with the Colorado Mined Land Reclamation Board. <br />Your notice must also state that anyone wishing to comment on the <br />application may view the application at the Alamosa County Clerk and <br />Recorder's Office and should send comments prior to the end of the public <br />comment period to the Colorado Mined Land Reclamation Division, Room 215, <br />1313 Sherman St, Denver, Colorado 80203. <br />After posting this notice you must provide a signed affidavit that such <br />notice has been posted according to the provisions of this Rule and submit <br />the affidavit to the Division. <br />Failure to submit the items listed above may result in a decision to deny your <br />application. You may amend the application by submitting all of the requested <br />items. In accordance with Rule 1.8, a new decision date will be established <br />90 days from the date of receipt of all requested items. If you have already <br />initiated newspaper publication, you will need to republish notice which <br />specifies the final date for receiving comments is the 20th day after the fourth <br />publication or the next regular business day. <br />
