C • • <br />Environmental Assessment Data <br />Omaha District, Corps of Engineers <br />Omaha, Nebraska 68102 <br />MROOP-N <br />The National Environmental Policy Act of 1969 (42 U.S. Code 4321 (1969)) <br />requires all Federal agencies to evaluate environmental values in any <br />Federally funded or authorized activity. In making this evaluation, <br />the initial, cumulative, and long teen effects of the activity must be <br />considered. The Corps of Engineers regulations state that in addition <br />to the completed application foam, the applicant may be required to fur- <br />nish such additional information or plans as may be necessary to assist <br />in the evaluation of the application. This information may include an <br />environmental assessment, possible alternative methods and sites, analy- <br />sis of material discharged and other information as may be necessary for <br />a completed evaluation of the application or preparation of an environmen- <br />tal impact statement. <br />Completion of this questionnaire as accurately and as completely as pos- <br />sible is necessary for us to be able to complete this evaluation. Failure <br />to provide adequate information will result in delays in processing your <br />pernit. <br />If other Federal, State, or local agencies are involved in this proposed <br />activity, it is possible that p~any of the items in this questionnaire <br />could be answered by attaching a copy of their environmental review or <br />environmental impact statement. <br />Even though some of the following questions may appear to be technical <br />in nature, quite often the applicants personal knowledge of the area will <br />more than compensate for any lack of technical knowledge. <br />1. Natural features of the immediate project vicinity can be ascertained <br />from photographs of the project site; therefore, these photographs are <br />very important. Applicant must submit color photographs with dates and <br />explanations of the views shown. <br />2. For projects which are publicly funded, the estimated cost of the pro- <br />ject and a description of how it will be Funded (i.e., Federal cost sharing, <br />municipal bonds, etc.). <br />3. Anticipated date that construction will be initiated January 1, 1984, <br />Date when construction will be completed 60 days from star rat rs, <br />the estimated beginning and ending date o construction activity in or <br />affecting any lake, waterway, or wetland.) <br />4. Description of Construction Methods. <br />a. Carefully describe the entire project in order to allow a thorough <br />evaluation of its environmental effect.
