Paynter -3- July 12, 1994 <br />EXHIBIT I - Proof of Filing with County Clerk: You must provide an <br />affidavit or receipt indicating the date on which the application was <br />placed with the local county clerk and recorder. <br />EXHIBIT J - Proof of Mailing Notices of Permit Application: You must <br />include proof of mailings of the "Notice of Permit Application" (forms <br />are contained in this application package) sent to the board of county <br />commissioners and board of supervisors of the local soil conservation <br />district in the application. We suggest you submit the date-stamped <br />receipt for certified mailing or a date-stamped copy of the notice form <br />with signature acknowledging personal service and receipt by the <br />appropriate board. <br />PERFORMANCE WARRANTY: Enclosed with your application package was a <br />document labeled Performance Warranty on Washington County letterhead. <br />The Division is returning your document and requests that you complete <br />and return the enclosed Performance Warranty document that is on State <br />of Colorado letterhead paper. <br />You may amend your application by submitting the requested items. In <br />accordance with Rule 1.5, a new decision date will be established 30 days from <br />the date of receipt of all requested items. If you have already initiated <br />publication, you will need to republish notice which specifies a 10 day public <br />comment period from the new date of newspaper publication. <br />This letter shall not be construed to mean that there are no other technical <br />deficiencies in your application.
