Open 14ining.Land Reclamation Act of 1973. The application may not be processed until
complete.
1. Submit 5 copies of all information including this form.
2. A fee of $50 + $15 per acre or fraction of an acre of affected lands.
3. A brief general description of the location of proposed (or existing) operation.

13. List, by number, current or previous permits held by the applicant(s) or

14. Documentation of the operator's legal right to enter and open mine the lands
