Preliminary review of the information received <br />deteralined that the following items must be received before the Division can consider your applicafion as being <br />submitted and technical review can begin: <br />APPi.iCATinN Ff1RM~ page 6, Signature Page: The Division requires that the ApplicanUOperator name be <br />the same as the name on the first page of the application (i.e. Whinnery Construction Co., LLC). <br />F.XAiRiT R -Site Deger' t=n inn ($yle fi.3.2/all: You must address Item (a) to the extent necessary to <br />demonstrate compliance with the applicable performance standazd requirements of Rule 3. At a minimum, you <br />must include a description of the vegetation and soil characteristics in the azea of the proposed operation. The <br />local office of the Natural Resources Conservation Service (NRCS) may provide you with this infomlation as <br />well as recommendations for Exhibit D -Reclamation Plan; <br />F.XHFRTT R -Site DeceriTnNon~(Rnle 6;_2(I~~ You must address Item (b) t0 the extent necessary to <br />demonstrate compliance with the applicable performance standazd requirements of Rule 3. At a minimum, you <br />must identify any permanent man-made structures within two hundred (200) feet of the affected area and the <br />owner of each structure. Permanent man-made structures include houses, hams, fences, above or below ground <br />utilities, imgation ditches, maintained or public roads, bridges, wells, pipelines, water storage impoundments, <br />railroad tracks, cemeteries, and communication antennas, etc. Each structure should be located on Exhibit E - <br />Map; <br />F.XAiR1T R -Site Descrjptinn nle fi3. (~: You must address Item (c) to the extent necessary to <br />demonstrate compliance with the applicable performance standazd requirements of Rule 3. At a minimum, you <br />must give a description of the water resources in the azea of the proposed operation. Identify any streams, <br />springs, lakes, stock water ponds, ditches, reservoirs, and aquifers which would receive drainage directly from <br />the affected azea. Provide any information available from publications or monitoring data on flow rates, water <br />table elevations and water quality conditions. <br />F.XAFRiT P - Reclamafinn Plan lRnle 63.41 (1) The purpose of the Reclamation Plan is to describe the <br />riming, procedures, criteria and materials that will be used to reclaim the affected land to the proposed future <br />land use. This plan must be correlated to Exhibit E -Map.
