A brief summary of these revisions follows. <br />MR 12 - Addition of Topsoil Stockpiles H and K and updated the <br />mining disturbance boundary; submitted August 6, 1998 <br />and approved August 17, 1998 <br />MR 13 - Updated the ownership and control information related <br />to SCC; submitted September 21, 1998 and approved July <br />21, 1999 <br />MR 14 - Updated the ownership and control information related <br />to SCC; submitted August 19, 1999 and approved October <br />28, 1999 <br />MR 15 - Updated Exhibits 12-1 and 13-2; submitted September 13, <br />1999 and approved October 12, 1999 <br />TR 09 - As-Built Report and Certification for Haul Roads A & B; <br />submitted February 9, 1998 and approved May 11, 1998 <br />TR 10 - Addressed increase in. reclamation bond because of <br />midterm review; submitted August 3, 1998 and approved <br />October 5, 1999 <br />TR 11 - Preliminary Development Activities of the Yoast South <br />Area; submitted May 27, 1999 and approved July 1, 1999 <br />PR O1 - Provided Life-of-Mine Plans for Mining and Reclamation <br />Operations; submitted March 1, 1999 and approved <br />October 15, 1999 <br />R.a is of S ;n~la ions <br />A total of 15 stipulations have been placed on Permit C-94-082 to <br />date. At the present time, 12 of the stipulations have been <br />resolved. The remaining three stipulations (#13 - Resolution of <br />KY civil penalty violations and bond forfeiture associated with <br />Permit Number 8980140; #14 - Resolution of WV Cessation Order No. <br />13 associated with Permit Number E012500; #15 - Pond 014 <br />embankment stability analysis) are still ongoing. <br />'i <br />
