The affected land azea (permit) covers about 3 acres. Most of the acreage is located in the <br />Elk Horn, Smoky Hawk, Blue Smoky and Wolf Fang mining claims, which adjoin each <br />other. Some of the proposed acreage is located about one and a half miles north in the Qui <br />-But No. 2 claim. The Qui-But No. 2 acreage is too far away to be considered the same <br />operation. Mr. H. Bruce Humphries, DMG Supervisor, determined that a second permit is <br />necessary to cover the Qui-But No. 2 acreage. However, Mr. Humphries will waive the <br />application fee for the second application. <br />2. Exhibit A -meets the required criteria, so it is OK. <br />3. Exhibit B - submitted a very detailed site description, so it is OK. <br />4. Exhibit C -looks real good. The applicant needs to submit a detailed description how the <br />affected land area (permit) will be delineated (marked) in the field, as required by Rule <br />3.1.12. <br />5. Exhibit D -excellent submittal. Just need two items: need the applicable quantities of <br />each grass & forb as pounds of pure live seed per acre and need you to provide the type <br />and quantity of equipment that will be used to backfill, contour and grade the sites. <br />6. Exhibit E -since you requested that the maps be held confidential the Division is <br />returning all the maps that do not apply to this application (Qui-But No.3 & No.8 and the <br />Cut Dog). Also, an application must contain a Mining Plan and Reclamation Plan map for <br />the public so you must re-submit the Mining Plan and Reclamation Plan maps, excluding <br />the confidential information, showing all the required information. Unless you decide <br />otherwise, the existing maps will be imaged into the confidential portion of the file (no <br />public access). <br />7. Exhibit F - OK. <br />8. Exhibit G - in order to comply with this Exhibit you must submit a document, which <br />shows you have a right to access the proposed acreage and conduct a mining and <br />reclamation operation. Copies of your mining claims or a copy of an access lease or a <br />notarized agreement from the landowner is okay. I assume these aze unpatented claims.
