This accident was reported orally <br />on July 12, 1996. <br />At approximately 9:00 a.m. on July 12, 1996, an employee of <br />Sunnyside Gold Corporation (SGC) was cleaning and opening: a road to <br />gain access to the Koehler Portal. This work is being done to open <br />the portal so the discharge water can be contained and ~.nonitored. <br />The outside edge of the road caved off under the weigfht of the <br />dozer and the dozer overturned, rolling one and one-h:ilf times. <br />The overturned dozer leaked diesel fuel and oil from the fuel <br />system and engine onto the rocks and ground in the vicinity where <br />it rolled and came to rest. The amount of this leakage is <br />undetermined but could range from 20-60 gallons depending on fuel <br />level at the start of the job. The location of the point where <br />this leakage occurred is at the headwaters of Mineral Creek in San <br />Juan County, approximately 1000 feet east of Highway 550 at the top <br />of Red Mountain Pass. The dozer came to rest on the slope above a <br />small pond. <br />The action taken to contain the leakage consisted of: <br />1) catching dripping diesel fuel until the tank could be sealed. <br />
