Cattany <br />Division Director <br />Natural Resource Trustee <br />.~ <br />Receipt of 110 Construction Materials Reclamation Permit <br />On April 23, 2004, the Division of Minerals and Geology received your the 110 Construction Materials Reclamation <br />Pemut Application package for the T Bone Stone Quarry, File No. M-2004-030. Preliminary review of the information <br />received determined that two (2) copies of the following items must be received before the Division can wnsider your <br />application as being submitted and technical review can begin; <br />F.XHTRTT F. - Ma nIP fi.i.51~ (I) In addition to the requirements of 6.2.1(2), you must provide a map that <br />clearly describes the features associated with the mining plan and the components of the Reclamation Plan. <br />Include one map for the mine plan and one map for the Reclamation Plan. The roap(s) must be drawn to a scale <br />no smaller than appropriate to cleazly show all elements that are required to be delineated by the Act and these <br />Rules; show a north arrow, note any section comers adjacent to the proposed operation, and indicate the date <br />illustrated. At a minimum, maps must include the following information: <br />(3) <br />(a) show the gradient of all reclaimed slopes (horizontat:verticat) sufficient to describe the post <br />mine topography; <br />(b) indicate where vegetation will not be established and the general azea(s) for shrub or tree <br />plartmg; <br />(c) if ponds aze a part of the Reclamation Plan, outline the final shore configuration of the ponds <br />and shallow azeas if the future land use is for wildlife; <br />(d) state the average thiclotess of replaced overburden by reclamation area or phase; and <br />(e) state the average thickness of replaced topsoil by reclamation azea or phase. <br />F.XA-RTT T _ Prnnf of Filing with (`nrmtp, (`Irrk (Rnlr (_,_'t_9~ Provide an affidavit or receipt irfdicating the <br />date on which the application was placed with the local County Clerk and Recorder.
