The assistance of Mr. Larry Perino, Reclamation Manager, was very helpful and <br />greatly appreciated. I have enclosed a copy of the inspection report for your records. <br />Violations <br />During the inspection, no deficiencies and no problems were noted. <br />Permit Issues <br />1. Sunnyside Gold Corporation has formally requested the "termination° of the industrial <br />Wastewater Discharge Permit for the Mayfower Mill, CDPS Permit # CO-0000426. <br />2. No discharge of wastewater was occurring during the time of the inscection. This discharge <br />has been inactive since 1990. The discharge pipe/conduit from Tailings Pond # 4 has been <br />sealed and grouted with concrete. Adequate documentation has been provided by Sunnyside <br />Gold Corporation. Please see attached documentation. <br />3. WQCD-Durango concludes that this discharge permit can be terminated. <br />Comments <br />The Consent Decree between Sunnyside Gold Corporation and the Colorado Department of Public <br />Health and Environment has been completed. Please see attached "Notice of Termination of Court's <br />Jurisdiction". <br />
