i -5- EXHIBITS A-J • Two (2) complete application packages must be submitted. One complete application package consists of the signed application form and the set of maps and exhibits referenced below as Exhbits A-J. Each exhibit must be presented as a separate section. Begin each exhibit on a new page. Pages should be numbered consecutively for ease of reference. If separate bound documents are used as appendices, please reference these by name in the exhibit. EXHIBIT A - Leaai Description and Location Map The legal description must identify the affected land, specify affected areas and be adequate to field locate the property. Description shall be by township, range, and section, to at least the nearest quarter-quarter section. A metes and bounds survey description is also acceptable. Also, the street address or lot number(s) may be used. This information is available from the County Assessor's office or U.S. Geological Survey (USGS> maps. NOTE: If you improperly locate your operation in th'e field, and as a result mine outside the area delineated by this permit application, it could be considered to be a violation of 34-32-123 or 34-32-124, C.R.S. of the Mined Land Reclamation Act. A map showing information sufficient to determine the location of the affected land on the ground and existing and proposed roads or access routes to be used in connection with the mining operation. Names of all immediately adjacent surface owners of record shall also be shown. The operation location map shall be a standard 1:24,000 scale U.S. Geological Survey map. The location of the proposed operation shall be shown and labeled with the mine site name. EXHIBIT B - Site Description Items 1-3 below must be addressed to the extent necessary to demonstrate compliance with the applicable performance standard requirements of Rule 6. At a minimum, you should include the following information: 1. A description of the vegetation and soil characteristics in the area of the proposed operation. The local office of the Soii Conservation Service (SCS) may provide you with this information as well as recommendations for Exhibit D - Reclamation Plan. Contact the Regional SCS office at 655 Parfet Rm E2000, Lakewood, Colorado 80215, (303> 236-2886 or the Mined Land Reclamation Division for assistance in locating the office nearest you; 2. Identify any permanent man-made structures within 200 feet of the affected area and the owner of each structure. Permanent man-made structures include houses, barns, fences, above or below ground utilities, irrigation ditches, maintained or public roads, bridges, wells, pipelines, water storage impoundments, railroad tracks, cemeteries, and communication antennas, etc. Each structure should be located on Exhibit E - Map. 3. A description of the water resources in the area of the proposed operation. Identify any streams, springs, lakes, stockwater ponds, ditches, reservoirs, and aquifers which would receive drainage directly from the affected area. Provide any information available from publications or monitoring data on flow rates, water table elevations and water quality conditions.
