Dear Mr. Fontanari:
On 2/10/97, DMG received response from Walt Eldridge, Map Surveys, Inc., to the
adequacy issues addressed in DMG letters dated 1/23/97 and 1/27/97. Thank you far
your timely response. On 2/10/97, DMG approved permit application M-96-076 for the
Western Slope Flagstone Quarry No. 2. Permit will not be issued until receipt and
approval of the financial warranty, in the amount of $3,078.
On 2/10/97, DMG received payment for the two copies of the Act, Rules and Regulations
previously forwarded to Mr. Eldridge and Mr. Fontanari. Thank you for the $16 payment,
in the form of check no. 5606. Please find enclosed, receipt no. 0425 for the payment.
