A11 immediately adjacent landowners of record mvst be shown. The euarter section corner, section line, or any other locatable points should also be shown on this map. EXHIBIT D - Wildlife, Water Resources, Vegetation and Soils Information Each of the above-mentioned subjects must be addressed as outlined in the applicable RULE. EXHIBIT E - Reclamation PZ As with the mining plan, this should be a general discussion of the methods, equipment, phases or stages of the reclamation process and those portions of Zand affected by each. If possible, the plan should be closely correlated with the map. If not, the narrative must be comprehensive enough to draw an accurate verbal picture of the expected appear- ance of the area after completion of reclamation. EXHIBIT F - Proof of Compliance with Local Government Zoning In accordance with 34-32-1I5(4)(e), the Board must make a finding that the operation will be in compliance with ZocaZ zoning or subdivision regulations. You are not required to submit this information, but a submission would assist the Division's pro- cessing of the application. when the Division notifies the local government of the existence of this application, it will request information regarding compliance with applicable local law. It is intended that operators may concurrently come into comoli- ance with local requirements at the same time the Division is processing this apolica- tion. Information regarding the status with Iocal government wi1Z be helpful. EXHIBIT G - Source of Legal Riaht to Enter This could be a copy of the Zease, deed, abstract of title, or current tax receipt. Rn acceptable alternative would be a statement acknowledged by a notary public that the operator has the Zega1 right to enter and mine. EXHIBIT X - Reclamation Costs For a IIO permit, if the operator requests financial warranty to be Zess than 55,000.00, the operator must submit Facts to support such a request. Othezwise, a financial warranty of 55,000.00 will automatically be set. For all I11 permits, the financial warranty shall be 52,500.00 per acre. EXHIBIT I - Terms of Governmental Contracts (lIl Applications only) For Special 10-Day (III) permits, the operator must submit a copy of the contract or other proof that proves the necessity of a special 10-day permit. The operator must also submit evidence of the financial warranty provided under the governmental contract, if it was required.
